Informed Consent

All research project consent forms must include the following language either by attaching the appropriate addendum to the project consent form or by inserting the addendum language, verbatim, into the text of the project consent form. The required language is provided in the consent form addenda links below. (Two versions are provided - one which includes FDA-related language and one with no FDA-related language.)
